How to Use Custom CIC Forms for Efficient Client Information Management
Custom CIC forms streamline data collection by digitalizing client information. This guide outlines the key steps to set up, manage, and review client information using custom forms in your Shortcuts
Setting Up Custom CIC Forms

Access the Console:
- Log into your Smart System Console.
- Click the Client Information tile.
- If applicable, select a site and enter your PIN to proceed.

Prepare the Form for the Client:
- Click on the Client Entry button.
- View available forms, including their last completion date and expiry details.
- Select the form, then hand the device to the client.
Client Form Completion Process
- Client Input:
The client fills out the form section by section, agrees to terms and conditions, and submits it by pressing Done. - Error Handling:
If mandatory fields are left blank, the system highlights them in red, ensuring all required fields are completed before submission. - Confirmation:
After submission, enter your PIN to validate the form.
Reviewing and Adjusting Forms

Employee Review:
- If the form requires a review, ensure all information is correct.
- Accept the form once satisfied.

Making Adjustments:
- Use the Adjust button to correct any errors.
- Hand the device back to the client for updates.
- The client must reaccept terms and press Done.
- Enter your PIN again to finalize the changes.

Canceling a Form:
- Use the Cancel button to discard forms with incomplete or incorrect information.
Additional Features

Edit Client Details:
Staff can update client information directly without needing PIN validation. -
Client History:
- View a comprehensive record of client services, purchases, and completed forms.
- Access specific forms or notes by clicking on their respective entries.

Visit Notes:
- Add notes for a client’s visit and save them for future reference.
- View cross-site histories by selecting the relevant option.
Navigating the System
- Access client history or return to the main screen using the Client Search button.
- Use the Show More option to expand the client’s history view.
